Letters of Support
CMS requires that Letters of Support from each participating facility be submitted with the project application. Letters of Support requirements and project commitment are outlined on page 5 of the CMP Application Handbook.
Please carefully review the requirements of the Letter of Support as outlined in the handbook. A Letter of Support template is provided here for applicant use. Applicants may use their own letters, the sample provided by CMS, or the templates provided here, as long as all of the essential components are provided in the Letter.
Note that corporate organizations that want to submit a single Letter of Support committing multiple nursing homes must include additional information as well as signed statements from those nursing homes. There is a separate template for "multiple facilities"

Project Participation
North Carolina provides a list of current CMP projects, their project categories, and the participating facilities. You are encouraged to view these lists before reaching out to facilities to avoid solicitating a facility that is unable to participate in your project. You are also encouraged to view the Denied Projects List provided by CMS to avoid duplicating a project that has previously been denied.
To search, open list and click Ctrl+F. Type the CCN into the search engine to find facility in the list OR you can search numerically. (Note that it is better to search by CCN instead of facility name, as name changes and changes of ownership may occur, but CCN will always remain the same)

It is important to note that nursing homes may be able to participate in more than one CMP-funded project as long as they follow guidelines implemented by CMS.
Facility Withdrawals from CMP Projects
Per the CMP Applications Handbook provided by CMS, "Nursing Homes are expected to honor their commitment for the entire length of the project. While circumstances may change during implementation, any withdrawal after a project has been approved compromises the project's integrity." CMS has placed the responsibility of monitoring project withdrawals on State Agencies. They have further indicated that once a facility commits to a project, that facility's funding is "locked up" for the duration of the project, whether it completes the project or not. Therefore, the facility is unable to participate in another CMP-funded project of the same type/category until the project it is committed to has ended. You can view the LIST of facilities that have withdrawn from projects and are unable to participate in the same type of project until after the project end date. NC Request for Applications
North Carolina DHHS/Division of Health Service Regulation offers the Culture Change Enhancement Grant through the North Carolina Culture Change Coalition. The RFA process can be found on the NCDHHS website, as well as the download link provided on this page.
Applications are submitted to North Carolina for review only. CMS is responsible for final approval of projects. North Carolina currently has an open application process. There is no deadline for application submission. Once applications are approved by CMS, the applicant/grantee will enter a contract with NCDHHS.
Please note that the RFA, which was released on July 1, 2025, is currently being updated to align with the QSO-25-26-NH guidance and the new CMS CMP Application. Questions should be directed to brandi.jordan@dhhs.nc.gov

CMS CMP Application
The CMS CMP application is the template that an applicant needs to follow when developing the CMP application. The applicant will need to ensure that each section of the application is completed, following the guidance of the CMP Applications Handbook.
Once an application is complete, it should be submitted electronically to brandi.jordan@dhhs.nc.gov
