NCCCC 2023 Star Search Nomination Form

PLEASE READ CAREFULLY!

Nominees must be employed with a certified nursing home in North Carolina and must have maintained consistent employment with the facility for a minimum of 2 years. There are two Nominee categories: Administrators and Staff.  Staff can include any staff member that is employed by the facility (he/she cannot be a contracted staff member of another agency that works in the facility).

  

Administrators can only be nominated by Regional/Corporate Directors.  Staff members can be nominated by any of their fellow co-workers.  Family members, residents, and others not employed by the facility cannot submit a nomination.  Nominations are due no later than September 29, 2023.  Winners and their supervisors will be notified prior to publishing on our website.

1st Place Staff and 1st Place Administrator -$500 gift card (each)

2nd Place Staff and 2nd Place Administrator - $300 gift card (each)

3rd Place Staff and 3rd Place Administrator - $200 gift card (each)
