Who We Are
Our Coalition Leadership team is comprised of our Chair and Vice-Chair, Lead Grant Reviewer for our CMP Grant Review Team, Secretary, Treasurer, and Committee Chairs. Our Active members are comprised of individuals who are responsible for carrying out the mission of the Coalition by providing outreach and education, soliciting membership, and generating ideas to continue to keep the concept of Culture Change thriving in our NC long-term care communities. The Coalition also houses the North Carolina Grant Review Team, whose primary responsibility is to provide review, feedback, and guidance to CMP applicants.
